Dr. Mario is the persona Mario takes in the Dr. Mario series of games. Under this identity, Mario dons a lab coat, a head mirror, and a stethoscope around his neck. He first appeared in his titular game, Dr. Mario. Dr. Mario and Nurse Toadstool, his assistant nurse, work at the Mushroom Kingdom Hospital, where Dr. Mario eliminates Viruses and other diseases by using Megavitamins, which has been the standard for subsequent games starring him. Dr. Mario has also appeared as an unlockable fighter in Super Smash Bros. Melee, Super Smash Bros. for Nintendo 3DS / Wii U and Super Smash Bros. Golden Eclipse.

- : Megavitamins - Dr. Mario fires a Megavitamin from his hand, similarly to Mario's Fireball. Megavitamins follow a unique bouncing arc that allows them to cover different angles. Unlike Mario's Fireball, they cannot be absorbed. (5.6% (early), 4.48% (late))

> : Tablet Toss-  He pulls out a tablet from his pocket before launching them. The damage and effect varies depending on its colour:

  • Red tablet: Does 10% of damage and causes no effects. Chances of appearing are 70%.
  • Yellow tablet: Does 15% of damage and some decent knockback. Chances of appearing are 15%
  • Blue tablet: Does 20% of damage and explodes soon after, dealing 25% damage and knockback. Chances of appearing are 15%.

^ : Pill Platform -  A giant pill appears beneath Dr. Mario, allowing him to land on it before jumping up into the air (like a trampoline). This move deals no damage but is probably one of the best recovery moves in the game. (0%)

v : Dr. Tornado -  Performs a multiple hit spinning attack with his fists outstretched. It also traps the opponent, hitting them constantly, before knocking them back. Dr. Mario can move around on the ground while spinning, and can hover in the air by repeatedly pressing the special button while using it. While the move has a start-up of 10 frames and has some considerable endlag, it has quite some range that allows it to beat out attacks. Gains KO potential at around 130% near the edge. Because of its high knockback growth on the last hit, it can be used as a risky but efficient edge-guarding option. (4% (hop), 20% (drop), 11% (landing))

F : Virus Frenzy -  Dr. Mario whistles, and suddenly, around 20 viruses appear from below. A few seconds pass by until the viruses begin to attack. When the Final Smash is finished, red viruses will disappear, blue viruses will explode and yellow viruses will stay on the battlefield as a common enemy. (3% (red virus), 4% (yellow virus), 5% (blue virus), 15% (blue virus explosion))
